29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a) Southbound Loop 101 at 90th Street on-ramp, Scottsdale, Arizona-On June 13, 2016, and at times prior thereto, NPL Construction Company, dba NBC did not instruct each barricade worker how to work next to traffic in a way that minimized their vulnerability, including but not limited to the consequence of contact between employees and motor vehicles.

29 CFR 1926.20(b)(2): The employer did not initiate and maintained programs for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers: a) Southbound Loop 101 at 90th Street on-ramp, Scottsdale, Arizona-On June 13, 2016 and at times prior thereto NPL Construction Company, dba NBC failed to conduct frequent and regular inspections of highway projects to ensure that employees did not cross active traffic lanes. Employees were exposed to crushing injuries from contact with moving motor vehicles.
