29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: a) Main Plant (south end of building): Employees were rigging a 21-ton lift bar with a Shepard Niles 30-ton cab-operated overhead crane and the lift bar was not secured properly onto the foundry hook causing the bar to become dislodged. Employees were exposed to struck-by and crushing hazards on or about June 17, 2016. ABATEMENT CERTIFICATION IS REQUIRED.
