On or about June 11, 2016 an employee disassembling the catenary scaffold fell breaking and/or shattering the bones of his back, ribs, leg, wrist, fingers and teeth.

Employers are required to provide fall protection for employees erecting or dismantling supported scaffolds where the installation and use of such protection is feasible and does not create a greater hazard.    a) On or about June 11, 2016 at the worksite located at CR-56 and Ohio-7 in Toronto, Ohio, Ohio DOT project number 96438, an employee disassembling a catenary scaffold fell over 14 feet breaking and/or shattering the bones of his back, ribs, leg, wrist, fingers and teeth. At the time of the accident the competent person had not provided horizontal lifelines, had not provided secure anchor points, and had not ensured the use of fall protection to protect employees from the hazard of falls.
