29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:    (a) The employer did not ensure that platforms and walkways on job made scaffolds used by an employee working on the cornicing of a two story house being built were at least eighteen (18) inches wide exposing that employee to fall hazards of more than eleven feet to the debris strewn ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:      (a) The employer did not ensure that each employee working off a job made scaffold approximately eleven (11) feet or more above the ground level below was protected from fall hazards.
