29 CFR  1926.416(e)(1):Worn or frayed electric cords or cables were used:  (a) On or about November 10 , 2020 at the jobsite employees were utilizing an orange extension cord where there was a orange extension cord which had the primary insulation damaged exposing the secondary insulated conductors. Employees were exposed to electrical and electrocution hazards.

29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   (a) On or about November 10, 2020 employees used an A-frame ladder that was folded to gain access to the top of the second story of the residential building and were exposed to a fall hazard of 11-feet.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) On or about November 10, 2020 and times prior thereto; employees performing framing operations were exposed to the hazard of falling 11-feet to the ground below.   The employer GRM Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1328141, citation number 1, item number 1 and was affirmed as a final order on August 9, 2020, with respect to a workplace located at 2002 Clearmist Way Royse City, TX 75189.
