29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   a. At the north side of the building, employees were exposed to a 14 foot fall hazard during stucco work, in that, a 42 inch wide scaffold had a single prefabricated metal platform 20 inches wide used as a working platform, on or about 06/20/2016.

At the north side of the building, employees were exposed to a 14 foot fall hazard during stucco work, in that, a single plank was used to bridge both sides of the scaffold to the roof to access the exterior walls of the second floor, on or about 06/20/2016.

At the north side of the building, employees were exposed to a 14 foot fall hazard during stucco work, in that, a tubular welded frame scaffold two frames high and two frames wide was used without guardrails on the top working platforms, on or about 06/20/2016.
