29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Maxham Farms Subdivision: 5959 Hidden Holmes Ln., lot 67, Austell, GA. Employees were installing roof shingles on a 6/12 pitch roof without using a personal fall arrest system and were exposed to fall hazards of approximately eighteen 18 feet to the ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Maxham Farms Subdivision: 5959 Hidden Holmes Ln., Lot 67, Austell, GA - Employees utilized a 32 foot aluminum extension ladder to access the upper landing surface of a residential house. The ladder was approximately seven inches above the upper landing surface, exposing employees to possible falls of approximately eight feet to the porch of the house below.
