29 CFR 1910.263(c)(3): Sprocket(s) and V-belt drive(s) of bakery machinery located within reach from platform(s) or passageway(s) or located within 8 feet 6 inches from the floor were not completely enclosed:    (a)  BASKET CONVEYOR, LINE 3 PACKAGING - The employer assigned temporary employees to manually place plastic baskets onto the Line 3 basket Conveyor Line, and the employer failed to ensure that all sprockets on the conveyor were completely enclosed.  This resulted in an employee injury where the employee's right thumb was amputated after it was caught between a sprocket and the conveyor, on or about June 3, 2016.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
