29 CFR 1910.335(a)(2)(ii): Protective shields, protective barriers, or insulating materials were not used to protect each employee from shock, burns, or other electrically related injuries while that employee was working near exposed energized parts: On or about 11 June 2016, maintenance employees were not wearing personal protective equipment such as gloves, face shield or other barriers when performing maintenance to the energized Beam Line 277/480V, 800 Amp breaker. Employees were exposed to arc flash burn hazards.

29 CFR 1910.335(b)(1): Safety signs, safety symbols, or accident prevention tags were not used where necessary to warn employees about electrical hazards which may endanger them as required by 29 CFR 1910.145: On or about 11 June 2016, the breaker in which maintenance employees were working did not have any warning signs or symbols on the outside or inside of the Beam Line Siemens 277/480V, 800 Amp breaker panel to warn of arc flash hazards. Employees were exposed to arc flash burn hazards.
