29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) On or about 06/06/16 at the Barilla American, Inc. located at 100 Horseshoe Boulevard, Avon, NY; employee was working on or near the gearing of Line 56 and was exposed to a caught in hazard when they failed to attached a lock to the group lock out on the disconnect for this equipment. NO ABATEMENT CERTIFICATION REQUIRED
