NOTE-Abatement certification and documentation are required for this item.

(a) On or about June 17, 2016 or at times prior thereto, employees were exposed to cave-in hazards at the base of a 6 foot 9 inch excavation wall while placing and erecting concrete forms, where no trench box, shoring, or shield was provided, or used.
