29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  a) Comfort Suites; 5236 Armour Rd., Columbus, GA 31904: The employer exposed two employees (roofers) to fall hazards while utilizing a JLG Industries, Inc. aerial lift (Model #450AJSII, Serial #0300108860) at a height of approximately 40 feet above the lower level without fall protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Comfort Suites; 5236 Armour Rd., Columbus, GA 31904: The employer exposed two employees (roofers) to fall hazards while sealing coping laps on the roof of the building at a height of approximately 40 feet above the lower level without fall protection.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)  a) Comfort Suites; 5236 Armour Rd., Columbus, GA 31904: Employees were required to work with chemicals such as, but not limited to Solargard Fluoro-Prime, Solargard 6083 Finish Coat, and Solargard 6083 Base Coat. The employer did not provide training specific to the hazards of chemicals used at the establishment to all employees working with hazardous chemicals.
