29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.  a)  Throughout  Warehouse 1; on or about June 15, 2016 and at times prior thereto the employer exposed employees to struck-by, slip/trip, and entrapment hazards in that employees were allowed to walk and work around aisles and passages that were not maintained in a clean and unobstructed condition.  b)   Thoughout Warehouse 2; on or about June 15, 2016 and at times prior thereto the employer exposed employees to struck-by, slip/trip, and entrapment hazards in that employees were allowed to walk and work around aisles and passages that were not maintained in a clean and unobstructed condition.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  a)  Throughout Warehouse 1; on or about June 15, 2016 and at times prior thereto the employer exposed employees to struck-by hazards in that employees were allowed to walk and work around improperly stacked insulation rolls and bags weighting up to 200 lbs that were damaged and routinely collapsed.  b)  Throughout Warehouse 2; on or about June 15, 2016 and at times prior thereto the employer exposed employees to struck-by hazards in that employees were allowed to walk and work around improperly stacked insulation rolls and bags weighting up to 200 lbs that were damaged and routinely collapsed.
