29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) Side of building, located at 220 Dellwood Drive Smyrna GA 30080: Employees were working from a pump jack scaffold at heights of approximately 19 feet. Employee used an inadequate fall arrest system by was wearing a body harness around his waist without the ring locate near shoulder level, or above the wearer's head. Employee attached life-lines to the carabineers was located at the wearer's waist.
