On or about June 15, 2016, at aisle 200 of the Warehouse, a defective 6-feet blue colored metal ladder being used by employees had not been taken out of service, exposing employees to a fall hazard. b. On or about June 15, 2016, at aisle 200 of the Warehouse, a defective 5-feet gray colored metal ladder being used by employees had not been taken out of service, exposing employees to a fall hazard. c. On or about June 15, 2016, at aisle 400 of the Warehouse, a defective 5-feet gray colored metal ladder being used by employees had not been taken out of service, exposing employees to a fall hazard.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. On or about June 15, 2016, at the warehouse, employees operating powered industrial trucks had not being provided with training that consisted in formal instruction, practical exercises and evaluation on the operation of power industrial trucks to prevent a struck-by accident.
