29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  a) On or about 6/22/2016, on the fly ash silo ladderway, the employer allowed employees to service a dust collector on top of the fly ash silo and did not ensure that a swinging gate was provided for the passage through the railing.   b) On or about 6/22/2016, on the ladderway platform on the central cement silo, the employer allowed employees to access the platform in order to conduct maintenance and did not ensure that a swinging gate was provided for the passage through the railing.   c) On or about 6/22/2016, on the ladderway leading to the top of the central cement silo, the employer allowed employees to access the platform on top of the silo in order to service a dust collector and did not ensure that a swinging gate was provided for the passage through the railing.  d) On or about 6/22/2016, on the ladderway on the north cement silo, the employer allowed employees to conduct maintenance and did not ensure that a swinging gate was provided for the passage through the railing.  e) On or about 6/22/2016, on the ladderway platform between the hopper and the fly ash silo, the employer did not ensure that standard railings and a swinging gate were provided.
