29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: a) Westway Terminal Company LLC, Philadelphia Terminal - The energy control procedures was not adequate to control the hazardous energy, in that the procedure did not indicate specific outline and technique to control hazardous energy, such as, but not limited to chemicals, etc., the specific steps to prevent the release of such energy and an outline to verify the hazardous energy is isolated. Employees conduct maintenance on the pipes. Condition noted on or about 1/29/2016.
