29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about June 22, 2016, the employer failed to ensure that an employee tasked with using an electric nail/stapler gun was wearing appropriate eye protection. The employee was wearing his normal prescription glasses only.

29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of the pallet dismantler machines: a) On or about June 22, 2016, the employer failed to provide any guarding for the Smart Products One Person Electric Dismantler, located inside the main shop. Employees are exposed to the approximately 20 inch saw blade that spans the width of the dismantler table when the machine is in operation. b) On or about June 22, 2016, the employer failed to provide effective guarding on the Smart Products Two Person Portable Gas Dismantler located outside in the work yard. Employees are exposed to the approximately 22 inch saw blade that spans the width of the dismantler table when the machine is in operation.
