29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). (a) 800 College Hill Road in Oxford, MS - On or about June 9, 2016, employees were using an aerial lift to access a roof and were using a climber's sit harness without attaching to the basket and used as a restraint system.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) 800 College Hill Road in Oxford, MS - On or about June 9, 2016, employees were on a steep roof, pitch greater than 4:12, without fall protection. Employees were not wearing a full body harness attached to a life line to arrest a fall. Employees were wearing a climber's sit harness that could be used as a positioning device to prevent them from reaching the roof edge but were not attached to a life line.
