On or about June 13, 2016, the operator of East set of Chester 3 ton Monorail cranes failed to concentrate on operating the crane without diverting attention to the West set of Chester 3 ton  Monorail cranes.     c. On or about June 21, 2016, the employer failed to ensure the Radio Frequency control actuator was legibly marked to indicate the function and direction of travel.

On or about June 13, 2016, the employer failed to ensure employees were clear of the below-the-hook lifting device on Kettle Crane #3 prior to beginning work in the clean-up area.
