29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es): Production shop, Torc-Pac 32-Ton press - on or about June 15, 2016, employees assigned to operate equipment for regular production stamping tasks were not provided with properly applied and adjusted single stroke two hand controls (point-of-operation devices) to ensure safe operation. INCH mode two hand controls were used for production stamping tasks while operators had their hands and fingers exposed to caught-in hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.217(e)(1): The employer shall establish and follow an inspection program having a general component and a directed component: Production shop, Torc-Pac 32-Ton press: on or about June 15, 2016, the employer did not establish and follow an inspection program having a general component, as specified by 29 CFR 1910.217(e)(1)(i)(A) through 29 CFR 1910.217(e)(1)(i)(C), and directed component, as specified by 29 CFR 1910.217(e)(1)(ii)(A) through 29 CFR 1910.217(e)(1)(ii)(C), to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment. Employees assigned to operate equipment for regular production stamping tasks were provided with inch mode controls due to malfunctioning single-stroke two hand controls (point of operation safeguarding device) since year 2015. Operators had their hands and fingers exposed to caught-in hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Operators had their hands and fingers exposed to caught-in hazards likely to result in serious injury.
