OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards. a) UAS #3 Hanger - On or about 6/22/16, employees were exposed to struck-by hazards during improper use of the Caldwell Rig Release in that it was missing the break-away chain. b)UAS #3 Hanger - On or about 6/22/16, employees were exposed to struck-by hazards during improper use of the Caldwell Rig Release in that the operator did not verify that the lever arm was in the fully closed position ensuring that the load arm was locked in position In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
