29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by paragraph (l):  Employees were exposed to struck-by hazards while operating a forklift (Manitou M230CP rough terrain forklift) in that the employer did not ensure that each employee had successfully completed the training.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  On or about 06/17/2016, the employer failed to report an amputation that occurred when an employee had his left thumb caught between two pipe flanges, while adjusting the pipes on a trailer.
