29 CFR 1926.303(c)(4): Portable abrasive wheels used for internal grinding were not provided with safety flanges (protection flanges) meeting the requirements of paragraph (c)(6) of this section    a) On or about May 10, 2016; First level of the scaffold system; employees were using portable grinders that were not equipped with guards while conducting pointing work on the concrete structure.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace    a) On or about May 10, 2016; First level of the scaffold system; employees were mandated to wear N95 respirators while conducting masonry work, possibly exposed to silica.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
