On or about June 22, 2016, an employee suffered injuries when clearing debris on a Kolberg-Pioneer crushing plant machine that was still in operation. (b) On or about May 23, 2016, employees were involved in servicing and maintenance on the rotor section of the Kolberg-Pioneer crushing plant machine and procedures for the control of hazardous energy was not established.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) The employer did not ensure that energy isolating devices were used when conducting servicing and maintenance. On June 22, 2016, an employee suffered serious injuries when clearing debris on the Kolberg-Pioneer crushing plant machine. (b) On or about May 23, 2016, employees were involved in servicing and maintenance on the rotor section of the Kolberg-Pioneer crushing plant machine and energy isolating devices were not used.
