29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:  a. On or about June 20, 2016, the employer did not preform a hazard assessment that would have identified the 115kV transmission lines in the area an employee was assigned to work in, exposing the employee to electrocution hazards.

On or about June 20, 2016, an employee operated an aerial lift while working within 10 feet of 115kV transmission lines, exposing the employee to electrocution hazards.
