29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    On or about June 23, 2016, employees were exposed to fall hazards while performing roofing activities on a residential roof and the employer did not ensure a competent person inspected the job site for hazards and implemented controls for the hazards.     Gabriella Almonte was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1145284, citation number 1, item number 1b and was affirmed as a final order on June 17, 2016, with respect to a workplace located at 7210 N. Gilles Road, Edwards, Illinois 61528.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)  On or about June 23, 2016, employees were exposed to fall hazards while accessing and exiting a residential roof via an extension ladder and the ladder did not extend at least three feet above the landing.  Gabriella Almonte was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1145284, citation number 1, item number 4a and was affirmed as a final order on June 17, 2016, with respect to a workplace located at 7210 N. Gilles Road, Edwards, Illinois 61528.
