On or about June 23, 2016 - Slitter Line, Knife Stand Area, employees required to enter the Knife Stand on the Slitter Line to make adjustments and/or set up the Knife Stand did not lock out the machinery prior to entering the hazardous area, thereby exposing employees to pinch points and lacerations. b. On or about June 23, 2016 - Baller Area, employees required to enter the Scrap Pit to insert trim scrap into the mandrel of the Baller did not lock out the machinery prior to entering the hazardous area, thereby exposing employees to rotating parts and lacerations.
