29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):      a) United Site Services dba Johnny on the Spot, LLC/Old Bridge, NJ facility- Throughout the facility: The employer authorized the use of powered industrial truck that had visible defects. Employees operated a Sellick forklift with a visible external damage to the front guard. The front guard on the forks was bent and cracked and truck was missing a cover to protect its battery compartment.    Condition observed on/or about June 22, 2016.

Employees transferred Super X, which is a corrosive from a 55 gallon drum into a spray bottle which was not labeled. The bottle was used throughout the shift by multiple employees.
