29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about June 16, 2016, at Lot 40A, 279 Oat Grass Drive, Grayson, Georgia:  An employee performing framing work while on the ground was exposed to struck-by hazards from falling nail guns, hammers, and lumber from the employees working on the house framing above him.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 16, 2016, at Lot 40A, 279 Oat Grass Drive, Grayson, Georgia:  Four employees performing framing work were exposed to fall hazards of approximately 13.1-feet to 21-feet when they worked from the unprotected edges of the house without using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 16, 2016, at Lot 40A, 279 Oat Grass Drive, Grayson, Georgia:  Four employees performing framing work were exposed to a fall hazard of approximately 10-feet when they used an aluminum extension ladder that was not extended three feet above the second floor landing.
