29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): On or about June 23, 2016, at the above addressed jobsite, an employee was working from the third level/platform of a supported scaffold, to remove/clean the excess concrete from a newly added exterior block wall of a commercial building, on which the platform was not fully planked, therefore exposing the employee to a fall hazard.
