29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      On or about June 24, 2016, employees were exposed to being struck by falling objects.  The employees were framing a newly constructed residential home located in Medina, Ohio 44256. The employee on the 2nd floor was working directly above two ground employees that were not wearing protective helmets.

On or about June 24, 2016, employees were exposed to being struck by flying objects.  The employees were framing a newly constructed residential home located in Medina, Ohio 44256. The employees were using air powered nail guns without wearing safety glasses.

29 CFR 1926.302(b)(3): All pneumatically driven nailers, staplers, and other similar equipment provided with automatic fastener feed, which operate at more than 100 p.s.i. pressure at the tool did not have a safety device on the muzzle to prevent the tool from ejecting fasteners, unless the muzzle is in contact with the work surface:    On or about June 24, 2016, employees were exposed to being struck by flying objects.  The employees were framing a newly constructed residential home located in Medina, Ohio 44256. The employees were using an air powered nail that had a safety device that was frozen in the open position an did not prevent fasteners from ejecting.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around them.    On or about June 24, 2016, employees were exposed to a fall of approximately 10 feet.  The employees were framing a newly constructed residential home located in Medina, Ohio 44256. The employees were working near and around an exposed hole in the floor leading to the basement area.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about June 24, 2016, employees were exposed to a fall of approximately 12 feet.  The employees were framing a newly constructed residential home located in Medina, Ohio 44256. The employee on the 2nd floor of the home was not using any form of fall protection.        The ET Construction Company LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 712238, citation number 1 item number 1 and was affirmed as a final order on 5/1/2013, with respect to a workplace located in Orville, Ohio 44667.
