29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:   (a) On or about June 25, 2016, Sebastian Rocha did not ensure that eye and face protection were used by employees when machines or operations presented potential eye or face injury. Employees of Sebastian Rocha were observed nailing shingles on a residential roof, using a pneumatic gun, without any eye or face protection. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about June 25, 2016, Sebastian Rocha did not ensure that each employee(s) engaged in residential construction activities 6 feet or more above lower levels were protected by guardrails, safety nets, or personal fall arrest systems. Employees of Sebastian Rocha were exposed to fall hazards in excess of 10 feet, while engaged in residential framing activities. Employees were not using fall protection equipment. 29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  (a) On or about June 25, 2016, Sebastian Rocha did not ensure that employees were provided with ladders where there was a break in elevation of 19 inches or more. Employees were exposed to fall hazards of up to 10 feet when descending from a top roof to a lower roof where the break was approximately 48 inches.
