29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  On or about 6/21/2016, at the above addressed jobsite, an employee was performing tree trimming operations without the use of a hard hat, was exposed to a struck-by hazard.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could contact cannot come closer to any unguarded energized overhead line.   On or about 6/21/2016, at the above addressed jobsite, an employee performing tree trimming operations at 5 feet 6 inches from a 7.6kV overhead power line that was not de-energized.

29 CFR 1910.333(c)(7): Portable ladders did not have non conductive siderails if they were used where the employee or the ladder could contact exposed energized parts:  On or about 6/21/2016, at the above addressed jobsite, an employee was performing tree trimming operations near a 7.6kV overhead power line from a 28 foot aluminum extension ladder, exposing the employee to an electrical shock.
