29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following: a. On or about June 24, 2016, at the above addressed jobsite, employees were cutting Exterior Fiber-Cement which contains the hazardous chemical silica and the employer did not develop and implement a hazard communication program which includes safety data sheets, thereby exposing employees to silica.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On or about June 24, 2016, at the above addressed jobsite employees were installing siding from a pump jack scaffold greater than ten feet above ground and the ends of the scaffold were not guarded, thereby exposing employees to fall hazards.
