29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Old Saw Mill, Throughout - On or about June 20, 2016 no energy control program, including machine-specific procedures, had been fully developed or implemented to protect employees from electrical shock or unexpected start-up while performing maintenance on the following: the Head (rig) Saw, the Log Carriage, the Edger, the two Chippers, the Trim Saws, the Roller Bed, the Lumber Conveying Chains, the Shaker Screen, the Vibrators, etc.
