29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. On or before June 27, 2016 where an employee operated the Uni-4 machine, the employee was not protected from the rotating sprocket and chain drive of the machine which resulted in an acute de-gloving amputation of the ring finger of the employee's right hand. b. On or before June 27, 2016 where employees operated the Uni-10 machine, the employees were not protected from the rotating sprocket and chain drive of the machine, thereby exposing the employees to a caught-in hazard.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. For the calendar year 2016 OSHA 300 log, the employer had not included all the information in column F as required for the following entries: a. Case 1 did not describe the injury or what object/substance that directly injured the employee. b. Case 2 did not describe the injury of the employee. c. Case 3 did not include the object/substance that directly injured the employee. d. Case 4 did not include the object/substance that directly injured the employee. e.
