29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:    a) On or about June 29, 2016 and times prior; blender platform; The employer failed to ensure all sides were properly guarded with top and mid rails, in that the blender side and loading side were missing mid-rails to prevent employees from falling up to 8 feet 7 inches onto the concrete floor below.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about June 29, 2017 and times prior; electrical lockout: The employer failed to ensure employees conducting clean-up and clearing jams on the blender were properly trained and locked out the 480v energy source prior to placing their hand into the drum to clean or clear a jam exposing them to the hazards associated with the unexpected release of energy.
