OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:  Location: 100 Morrissey Blvd Boston, MA  On or about June 23, 2016, an employee was injured when the pipe testing cap struck the employee in the head.

29 CFR 1926.200(a): Required signs and symbols were not visible when work was being performed:  Location: 100 Morrissey Blvd Boston, MA  Warning signs were not installed around pneumatic testing area where the 16 inch pipe was being pneumatically tested to keep employees not involved in the testing out of the area.
