29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about 24 June 2016 and at times prior thereto, two employees were performing framing operations on a residence under construction without eye protection, exposing the employees to the hazard of being struck by falling or flying objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 24 June 2016 and at times prior thereto, three employees without fall protection were performing framing activities on the second floor of a residence under construction, exposing the employees to the hazard of falling approximately 12 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about 24 June 2016 and at times prior thereto, the employer did not provide fall protection training to three employees performing framing activities on the second floor of a residence under construction, exposing the employees to the hazard of falling approximately 12 feet to the ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  On or about 24 June 2016 and at times prior thereto, three employees performing framing activities, accessed the second floor of a residence under construction by using top step of a 6 foot stepladder, exposing the employees to the hazard of falling approximately 6 feet to the ground below.
