29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) At the northwest corner of the residence where sinkhole activity was to be measured, employees were allowed to conduct drilling operations near overhead power lines without ensuring the lines were deenergized or covered before work began. The 30 feet long drill rod contacted an energized 7,620 volt overhead power line resulting in the death of one worker and the severe injury of a second worker, on or about 6/28/2016.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a) Employees conducting drilling operations at the northwest corner of the residence had not been instructed on the hazards associated with operating the drilling rig near overhead power lines and how to control or eliminate the electrocution hazard associated with contacting the power lines.
