29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) 1544 Boone Avenue, Bronx, NY - Building A - 2nd Floor:   Employee spraying Isolatek Blaze Shield II Fireproofing was required to wear respiratory protection (e.g., N95 respirator) and the employer did not establish and implement a written respiratory protection program; on or about 8/15/16.    b) 1544 Boone Avenue, Bronx, NY- Building A - 1st Floor:   Employee mixing Isolatek Blaze Shield II Fireproofing was required to wear respiratory protection and wore a half-face air purifying respirator with P100 filters.  The employer did not establish and implement a respiratory protection program; on or about 8/15/16.
