29 CFR 1910.119(l)(2)(iv): The written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affected a covered process did not address the necessary time period for the change: (a) On or about June 14, 2016 for employees potentially exposed to fire and explosion hazards where the company replaced a measurement orifice with a flange without properly closing out the management of change for the device installed at the K-101 unit in the facility at or near 3815 116th Avenue SW, Dickinson, ND 58601.
