29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 29, 2016, employees were exposed to a fall of approximately 9 feet. The employees were installing roofing materials on an existing residential home located in Brunswick, Ohio 44212. The employees were using an extension on the west side of the home to access the roof. The ladder did not extend at least 3 feet over the landing area of the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about June 29, 2016, employees were exposed to a fall of approximately 19 feet. The employees were installing roofing materials on an existing residential home located in Brunswick, Ohio 44212. The employees were working on all areas of the roof without wearing fall protection. The Aquaproof Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 990805 citation number 1, item number 1 and was affirmed as a final order on 10/9/2014, with respect to a workplace located at 3688 Fenn, Medina, Ohio 44256.
