29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  (a) By the Baler Area, affected employees would clean the inside of the baler without receiving the authorized employee training exposing employees to accidental/inadvertent start up of the equipment.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   Employees who perform cleaning activities on and around the baler were not required to:  (1)  know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,  (2)  turn off and shut down the machines  (3)  physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4)  affix lockout or tagout devices to each energy isolating device,  (5)  prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.   (a)  Employees were required to clean out the International Baler Corp Two Ram baler when performing grade changes without perform the control of hazardous energy (lockout/tagout).  This exposed employees to struck by and crushing hazards.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:   (a)  The employer failed to evaluate the forklift operators at least once every three years by first observing the powered industrial truck operator during normal operations to determine if the operator was performing safely, and second by asking pertinent questions to ensure that the operator has the knowledge or experience needed to operate the truck safely.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  In the Baler Area; employee(s) would walk up a conveyor in order to grab unwanted product from the line before it went into the International Baler Corp exposing employees to caught in hazards.    (b)  Prior to June 28, 2016 in the Baler Area; employees would clean the underside of the conveyor were exposed to struck by hazard when the conveyor with cleats went past the supports of the conveyor system.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   (a) Prior to June 28, 2016, the Black Clawson (S/N:  245) guillotine shear was not guarded at the point of operation exposing employees to struck by hazards.  The A Greener Vision Recycling was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 329439, citation number 1, item number 2b and was affirmed as a final order on May 16, 2012, with respect to a workplace located at 2350 W. Packard St., Appleton, Wisconsin.
