29 CFR 1926.416(a)(3): When and where energized circuits existed, the employer failed to post and maintain proper warning signs, advise employees of the location, hazard involved, and the proper protective measures to be taken:    a) Jobsite: Hip City Veg space, mezzanine - Employees conducting demolition of electrical wires were exposed to shock hazards as the employer did not post and maintain proper warning signs, or advise employees of the location of the energized wires, the hazards involved, and the protective measures to be taken.  An employee who cut an unidentified energized electrical wire with a wire cutter was shocked and hospitalized.    Noted on or about 6/16/16.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.    a) Jobsite: Hip City Veg space - An employee conducting demolition activities on the concrete mezzanine, 8 feet above ground level, was exposed to fall hazards without the use of a fall protection system.
