29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):          a) Busway Plugs/Plug Assembly Area, 168 Georgetown Road, Canonsburg, PA 15317:  On or about June 22, 2016, a Komax brand automatic cutting machine (model number 206, serial number 206-0155) that was being used to cut shrink tubing was not equipped with a discharge channel and Plexiglas safety cover.  The discharge channel and Plexiglas safety cover prevent machine operators from accessing the area of the machine where the blade is located.  Employees operating the Komax brand automatic cutting machine were exposed to cut-by hazards.
