29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   a) Bradford Hotels, LLC DBA Holiday Inn Express Bradford- On or about June 30, 2016, and times prior thereto employees were working with and around hazardous substances including, but not limited to, Ethanedioic Acid Dihydrate, Sodium Hypochlorite, Potassium Hydroxide while performing housekeeping laundering duties. Facilities for quick drenching or flushing of the eyes and body were not available to employees working in the laundering area.

29 CFR 1910.1030(h)(1)(ii)(B): The medical record for an employee with occupational exposure did not include a copy of the employee's hepatitis B vaccination status, including the dates of all the hepatitis B vaccinations or any medical records relative to the employee's ability to receive vaccination as required by 29 CFR 1910.1030(f)(2):  a.) Bradford Hotels, LLC DBA Holiday Inn Express Bradford- On or about June 30, 2016, the employer had not obtained records of employees' hepatitis B vaccination status.  Employees were exposed to blood or other potentially infectious material during laundering and housekeeping operations performed throughout the facility.

Bradford Hotels, LLC DBA Holiday Inn Express Bradford- On or about June 30, 2016, the employer had not obtained signed declination statements from employees who declined to accept hepatitis B vaccinations. Employees were exposed to blood or other potentially infectious material during laundering and housekeeping operations performed throughout the facility.
