29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: a) On or about June 30, 2016 and at times prior, employees were required to use a 3M 7502 half face respirator while spray painting. The respirators used by the employees were left in the paint storage area and were not packed or stored to prevent the contamination of dust and chemicals.

29 CFR 1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle: a) On or about June 30, 2016 and at times prior, employees were required to use an LP-gas container on a Toyota forklift. The safety pressure relief valve of the container was not positioned on the vapor source (top). The safety relief valve was positioned at the bottom of the container. Note: ANSI B56.1 - 1969 refers to NFPA 505 - 1969, Part C, Section 602.L. in which the handling of removable LP-Gas containers shall be securely mounted and be positioned so that the safety pressure relief valve opening is always in contact with the vapor source (top) of the container. This is accomplished by an indexing pin and container clamp which, when the container is properly installed, positions the container.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure they were readily accessible during each work shift to employees when they were in their work areas: a) On or about June 30, 2016 and at times prior, employees were required to use hazardous chemicals in the workplace. The safety data sheets of the hazardous chemicals were not readily accessible to the employees including, but not limited to: 1) Spot check SKD-S2 - contains propanol, propone, and isobutene 2) Cardinal semi-gloss paint - contains titanium dioxide
