Grafton Ready-Mix Concrete, Inc. located at 1155 Elm St. Lorain, OH; On or about June 8, 2016, the employer did not ensure employees were provided the information contained in Appendix D of the respiratory protection standard to employees who wore filtering face piece respirators (dust masks) voluntarily.
